i found pulling my knob didn't work, i had to take tools to mine. Not it's in pieces.
Got a chrome one on there now that fits using a simple grub screw like Fwaza said.

I cut mine with a stanely knife after pulling for hrs! Just cut it along the indented lines and it will break off a lot easier then.

Wee screw(s) in the side of the lever that screw directly onto the shaft to hold it tight against it... Usually come loose so when tightened a wee bit of glue doesnt go amiss...
Richbrook stuff comes with screws and allan key anyhoo...

With mine i just got a rubber grippy mat and used that and just twisted it of with brute force lol
