get a copy from where mot was done.... 
they must keep a record...
don't know if you can though

ring up dvla or something & ask them about it
it will be on there records of where it was done & you should be able to go get a duplicate from that garage...
about £12-£15 I think tho... not sure

if u go 2 the garage who done it the can give u a duplicate copy
