The stunt is real.. Its very possible, been done loads of times before and its not like the car isnt cheap and disposable.
As for was Clarkson in the car? Ofcourse he wasn't..
I would imagine the whole reason for "battering" the car before taking it into the sewer to do the stunt was so you wouldnt notice the damage when they did several takes. i.e. the car was already damaged so if they fluffed up the first attempt you wouldnt be thinking "why is that head-light smashed and the wing bent?".

also as said - different bit of tunnel, filmed on different day, with different drivers, that may even be a different car.
