I work in a takeaway so I get free food from there but I work out at the gym lots of times a week to try counter it out.
My girlfriend takes things like tuna, pasta, mayo to work and things like that that she makes before she leaves.

When I do a 9 - 12 hour shift at Tesco I normally dont start till 12/1pm, which means I tend to, at the moment, have half a pizza before I go, take the rest of it with me and eat that half around 7pm with drinks of milk and squash throughout the day. Kept busy so dont really get hungry, which is good in a way. Therefore it costs me the price of the pizza and the cost of cooking it, so on average £2.50 should say.
On the commute front I use my RS <--
Dont get stopped by traffic, and it costs me about £9 every 2 weeks to fill up

When the weather was nice, I was tempted to ride my Aprilia RS
However, 90 miles a day on it is a bit much.

As much as i pains me, i can't find the fail either. Apart from the fact that you should have some fruit or veg in there.
Edit: I'm not working today, but when i do work, i normally have some tuna or chicken salad sandwiches (home made), and an apple.

I take sandwiches with me and a chocolate bar. Then i buy either, Burger king, KFC, McDonalds, Fish & chips, nandos or get a dominos delivered.
I'm trying to cut down as some weeks i spend ridiculous amounts.

Cup-a-soup and a cereal bar for lunch, couple of pieces of fruit for snacking. Seem to be getting less and less time to eat at work

I never get chance to eat at work, leave the house at 5am and often the first time I eat will be 8/9pm when I get home. Hence I'm a skinny runt.

Tuna Pasta (or pasta stacked with tomatoes and cucumber)
or
Chicken/Beef with Salad Wraps made at home.
Along with a Yogurt.
Used to buy lunch everyday from Greggs, but noticed significant saving making it myself.

I'm also finding i'm spending too much on food at work and started to put on weight. My last role i wouldn't east some days and it was a lot more active.
Find if i buy food in to make stuff i either end up eating it all or i cba.
Think my best bet is to visit Tesco before work and just leave 10 minutes earlier.

May be more than that
I need to fill my car every 3.5 days. (so 3 unless I plan on not returning from work).
Each time I fill I put about £40-£50 in ~ £11.50-£14 a day in fuel I need a diesel
P.S. Doyle the fail you are looking for, is your life.
I've been doing this for 2 years, hence my constant moan about fuel prices.. I now have the joys of paying another 2.80 a day to use the Mersey Tunel.

Yeah I've always found that too... If I buy wraps, fresh chicken etc... I make one or two wraps and then leave it all too long and everything goes off / stale ... then i chuck it all in the bin.

Sandwich shop next door will do a sandwich or a cob with a massive filling, can of arab Pepsi and some crisps or fruit for £1.99; or can have a baguette, crisps and a bottle of Coke for £2.80.

I work from home, so I basically have access to anything, I try to eat healthy things like pasta based dinners or salads but if I'm doing a bit of driving around the sites I just nip into Subway or whatever, Subway is probably my fave instant food store.

I work offshore so just basically go to a canteen for lunch
Usually either soup or a meal. However, I would prefer it if there was the option of a sandwich or some sndwich fillings
