He could have bought it with a fucked thread, I'm assuming they'd do at least a simple service before heading out into the middle East. The thread would have been more damaged when they removed the old plug and more damaged when they re-fitted the new plug to the point that it was probably just on the very edge waiting to come out. The constant huge pressure of the combustion chamber would have been more than enough to force it out after a while.
I'm guessing that then when Clarkson says 'a wee Punjabi cunt stayed up all night with it' or something that they had replaced the head.
