Had the same thing last year. It's issued by your old company and will be sent to whatever they've got on file. Whitter is right - they send it to you to be checked before you get it issued. It'll show all the taxable benefits you had.

Double check it as one year I had a Range Rover for 4 weeks on my P11d that I certainly didn't have - would liked to have know which cheeky badger did though.
