Adrian flux have requested one of the above forms, what is it?! I cant remember what they said on the phone but sure it was just something saying what mods have been done and my signature at the bottom? or is it something from a garage to say the mods are safe?! thanks in advance

Oh that reminds me I need to do it, iirc you just fill it out sign it and send it, was supposed to do mine last month but I forgot

I dont know if they do the same as Brentacre but they sent me a form through the post to fill out, sign and send back to them. I'm sure my mate who's with flux had to do the same.

Yea you should have been sent the form with all your paperwork,just fill in what mods you have and sign it and send it back
